Breast Cancer — My Story

The reminder that my mammogram was overdue sat in my ‘in’ box for over a year. Finally, one day I got sick of putting it off, pulled it out, and called for an appointment. The day of the test, the technician was very friendly, chit-chatting as she took the pictures, but as she looked at the results on her monitor, the chatting stopped, and she seemed more reserved.

I went home and told my husband that I felt unsettled about the test, and that I would not be surprised if I got a call about the results. The call came the very next day. The doctor described the results as ’suspicious’ and asked me to come back in for more pictures and a biopsy. That accomplished later in the week, the next call (the one I dreaded) came telling me that I had a large area of DCIS, which means “Ductal Carcinoma In Situ”, an early cancer. Since the cancer was stage 0, I had a large lumpectomy. When the lab work came back, they found that not only was there more cancer left in the breast, but on a scale of 1-9 for ‘badness’, mine was a 9. So the surgeon gave me the option of another lumpectomy, (the normal approach for DCIS) or, considering the size of the area and its potential for nastiness, a mastectomy. I struggled with that decision for many days, and glued myself to the web, searching for answers. Ultimately, I decided to have the mastectomy, with immediate reconstruction.

My research told me that I could hope to have a ’skin-sparing and nipple-sparing’ mastectomy, with reconstruction using fat from my stomach. So I hopped on the Web and found Dr. Robert Allen in New York City who had pioneered the procedure of fat-only transfer to rebuild the breast. This is more difficult than the transfer of muscle, because the blood vessels in fat are more minute than those in muscle, requiring extra training in microsurgery to carry out.

On the whole, I am happy with the results. The shape of the breast itself is great, and the fat transfer was successful, which is no small accomplishment in itself. Unfortunately, the nipple/areola did not do well, due to a blood clot underneath it after surgery, which was not caught. After a year of healing, it looks pretty good, and with more surgery I think it will end up fine. The abdominal scar, on the other hand, is 20″ long and there is no other way to describe it but ugly, although I hope it will get better as the scar fades.

Looking back, I think I might have done things a bit differently. I let the first surgeon scare me with the statistics of how likely the cancer was to return without a mastectomy. The reality is that had I had another lumpectomy that got all of the cancer, that would probably have been enough, with radiation, to give me the same survival rate that I have now, having had a mastectomy without radiation. It turned out that there was not much cancer left after the lumpectomy — another pass would probably have gotten it all, plus a nice wide margin around the cancerous area, which is the major determining factor of whether this particular cancer will return. A wide margin is key, and far more meaningful than any statistics. I wish I had made that second pass and assessed the results. If I had concerns at that time, I could always have continued on with a mastectomy. There is no going back, however, once the mastectomy is done.

Here are some thoughts to consider:

1. No doctor will care about the outcome of your situation as much as you do. And no doctor will have to live with the outcome — you will. Every doctor, as caring as he might be, still has his own agenda, potentially in conflict with yours, simply because surgery is how they earn their living. So listen to your doctors along the way, but be your own advocate, and make your decisions about what is best for you. Learn about what is available, and go after what you want.

2. Be aware that Federal law requires your insurance to cover reconstruction. They must also cover the matching of the other breast, if need be, so that you end up with a “matching set”. And many insurance HMOs or PPOs will allow you to go elsewhere if you can prove that your group does not offer the procedure you want.

3. If a lumpectomy is offered as a treatment, consider exhausting that option first, before jumping into a mastectomy. Sometimes there is no difference in survival rate between the two, and your doctor can better advise you if this may be the case for you. As I mentioned above, the margin around the cancer is a major factor. One big difference between most lumpectomies and a mastectomy is that there is no feeling left in the breast after a mastectomy, which can be very disappointing, although sometimes some sensation will return after a mastectomy. On the plus side, you may be able to avoid radiation by having the mastectomy. Radiation is generally a requirement when having only a lumpectomy.

Naturally, getting rid of the cancer is Job One, but consider all of your options, and don’t just take the most radical approach if you don’t have to, to achieve the same survival rate. Consider your quality of life, as well, if you have options, and don’t be ashamed for doing so. Having a mastectomy and reconstruction will be one the most difficult things a woman ever goes through, and it is not a procedure to be done for any other reason than a woman is certain it is best for her in every respect. The lumpectomy, on the other hand, is easier to get through, and you are on your feet and well in a matter of days, breast intact. Please don’t imagine I am suggesting that you take the easy road, when it might compromise your survival. Just think twice if there is no statistical difference in survival between lumpectomy/radiation and mastectomy.

4. Go after the latest procedures, and go to the best doctors you can. Nipple sparing mastectomy is relatively new, so if you want that, then find a surgeon who will do it for you. Go for a fat-only transfer, the most common of which is called a DIEP flap, the one developed by Dr. Allen. I also see that his practice has added a partner that specializes in nipple sparing mastectomy since I saw them for my surgery, so they are worth a visit at diepflap.com. Or try Sloan-Kettering — they seem to have the most comprehensive overall approach to breast cancer reconstruction, from all of my research, and will offer similar options. Lastly, it is also sometimes possible to transfer a nerve along with the fat, which may improve the chances of regaining some sensation in the breast, so ask your surgeon about this.

5. Question all incisions. I stopped the first surgeon and asked him if he had to put the incision right in the middle of the inner half of my breast, where I would always see it. It turns out he didn’t, it was just easier there. So don’t worry about ‘inconveniencing’ your surgeon for a few minutes, when you might have an unnecessary scar to look at for years. Question everything.

You may also want to inquire about having a ’sub-cutaneous’ breast reconstruction.This is where the incision is made along the crease under the breast. Surgeons currently prefer going in through the nipple area, but to me, cutting around the nipple, especially if you want to keep it, can only compromise the blood flow, putting the area at risk. They will tell you that a sub-cutaneous incision makes reaching the upper area of the breast tissue harder, but if you have a larger, sagging breast, a very large (6-7 inch) incision could be made without ever being seen again. An incision this size would likely allow a surgeon to reach any part of the breast area. There are a few surgeons doing this, but new techniques are slow to catch on. This option, of course, wouldn’t make sense for those with small breasts, where the large incision would show.

6. There are new reconstruction techniques just around the corner. Surgeons today are sometimes reluctant to do a very large lumpectomy or quadrantectomy (where one-quarter or more of the breast is removed) because it will disfigure the breast, and the reconstruction of these irregular shapes is harder with current fat transfer technology, although there are a few surgeons who will do this. More often, though, if too much breast tissue needs to be removed, a surgeon will suggest a mastectomy instead, because creating an even breast-shaped mound is, simply, easier. Improved methods in fat transfer are around the corner, however, which will make it easier to fill in dents and irregular areas.

The most promising method, using liposuction to remove fat cells from another area, mixing them with stem cells, and then injecting this mixture into the irregular breast area, may someday become a preferred method for reconstruction. It may become feasible, for some cases, to utilize this method to achieve that wide margin around the cancer, while reducing or eliminating the need for a large scar, and preserving a more natural breast. At least one company, Cytori, is making headway on this. Fortunately, breast cancer treatments and surgeries are improving quickly, and in another 10 years, I believe the approach to treating breast cancer, on all fronts, will be very different from what it is today.

Packing is the one aspect of moving that most of us despise. It is also the aspect that takes up most of the time involved with a move. And while we’re busy trying to figure out how to find enough time to get our packing finished, it also begins to dawn on us that some things that some things are more difficult to pack than others.

A good example of packing and the phenomenon of “this is harder than it looks” is trying to get pots and pans from the kitchen ready for the big move. This article provides the basics of getting yours ready quickly, safely and easily.
Here are the basic pots and pans packing tips:

Pots and Pans Packing Tip 1: Get the right materials together.

Fortunately, the first step isn’t difficult. Here’s what you’ll need:

• Medium-sized, sturdy moving boxes
• Packing paper, kraft paper, or in a pinch, newspaper
• Packing tape
• Dark colored, wide-tipped marker
• Biodegradable packing peanuts (optional)

Pots and Pans Packing Tip 2: Prepare your moving boxes.

Take each box and tape the bottom both lengthwise and crosswise along its seam, forming a cross. And while this may seem like a ridiculous reminder, be certain that you have the bottom of the box fully taped and secured before you start putting things in- you’d be surprised by the number of people, under the stress of the move, who forget to do this!

Next you’ll want to prepare the inside of the box. Take the paper and form very loose “balls”- making them too tight will reduce their ability to absorb impact and will also exhaust your paper supply very quickly. You want to be sure that you have at least 1 – 2 inches of paper plus biodegradable packing peanuts to create a cushioning layer at the bottom of each box.

Pots and Pans Packing Tip 3: Loading the pots and lids.

The first thing to keep in mind when loading the pots and lids into the boxes is that pots get nested together in groups of 3, and the lids for the same 3 pots get nested together as well. Then you’ll place the group of 3 pots and the group of 3 lids together in the same box. This way, when you’re unpacking, you won’t have to tear through all the kitchen boxes to get a single pot and lid that match.

When nesting the pots together, it’s usually easiest to start with the largest pot, wrapping it in about 3 sheets of paper and taping the corners of the paper to one another inside the pot. Once the first pot is secure, load the second pot into the first, wrapping and taping it the same way. The third and final pot of the group should follow the same procedure.
If you have biodegradable packing peanuts, put a single layer between each pot.

When labeling the box, try following these additional guidelines:

• Mark the room on the box first, with “KITCH” for “kitchen” and a number for each box (professional packers would take care of this for you).

• Label the box on at least 2 sides (all 4 is best) and the top so you can always quickly identify what’s inside when moving and unpacking. This saves lots of time during the move.

• List the contents on the sides as well to avoid confusion later.

• Mark “Fragile” on boxes containing glass or delicate items.

• Use a dark, broad-tipped marker for labeling the boxes.

• Use block print in letters large enough to read from 6 feet away.

• Keep one single kitchen box as an “Open Me First” box containing the things you’ll want to use within 24 hours of arriving at your destination. Be sure to mark it with “Open Me First” in big red letters.

A Quick Word Helps to Remember

Do you have a list you have to remember quickly? Take the first letters and form an acronym. For example – you have to remember to send Fred, Lisa, Ethel and Andrew cards. Yes, their first initials form the word FLEA. Imagine itching (your head?) because you have a flea – you know the first letters of their names. Going to the supermarket? Bread, vinegar, ketchup, onions, tuna, olive oil- BVKOTO. Not too promising? Try rearranging them – you have TV BOOK. Think of a book jumping out of a Television and attacking you. Silly? Yes. But remember – the sillier the better, it will force it to stay in your head. Now you have all the letters. If you don’t have enough letters to form a word – try to find one that comes close. PROMPT for PRMT, FAULT for FLT and so on. You already know the items you have to remember. What you are trying to do is create a reminder – once your memory is jogged, the words will come back. When you are devising the acronym, remember to picture the items visually in your head, visualization is a strong memory technique.

Forming A Story

If you have a longer list of seemingly unrelated items, that automatically becomes a good candidate for a story. For example:

Glass, Horn, Cat, Onion, Melon

These words may be unrelated, but that is the point – you can combine them easily with a story that will be outrageous enough, it will stay in you head – change the order if necessary – A cat is playing with a melon and all of a sudden it’s repelled by the smell of an onion that was used to season the melon. The cat got all excited and ran away – racing through a glass window and landing inside a big French horn. It’s silly and childish – but that’s the point, it’s silly enough to be remembered. Think the story through and the key words – cat – melon – onion – glass – horn – come to mind. Of course, it’s unlikely that you’ll ever have to remember a list of words such as this, but the point is not to be afraid to create the absurd. It works.

Longer Lists

What if you have an extremely long list of items to remember? Try breaking it down into more suitable sub-chains and apply the same rules as above. Better still – see if you can organize the list better. Put similar or like items together. Are you able to form acronyms from any of the items? Can you separate the list in such a way that the acronyms form words and then you can combine these words? Remember, the more ridiculous the thought – the stronger the impact and the better it will stay in your memory. It helps if either the acronyms or the associations you create can form mental images. What you can see in your mind is very strong and the stronger something is, the better you can play with it and the easier it will be to remember. What may be difficult at first will come with practice.

Relate the Facts

If a piece of information doesn’t relate to you, then try finding something about it that does. For example, the human body has 60,000 miles of blood vessels. Kind of hard to believe? Think that means ten round-trip drives between Los Angeles and New York and it will take on meaning. Do you have to remember the physical dimensions for a room or a field? Compare it to something you know already. If you’re a sports fan, chances are you can picture the size of the playing field. How does that compare to the space you are trying to remember – bigger smaller, would it fit into a football field – how many times?

Exercise & Motivation, Part 2: Overcoming Inertia & Getting Started

In the first article in this series (http://tinyurl.com/8ztbo), we gave you an overview of the stages of change in Prochaska’s Transtheoretical model. The first three of these stages are all about getting started. They apply to anyone who’s not actually regularly exercising at the moment. If you were active in the past, but don’t now (i.e. you’re in the “Relapse” stage), then, motivationally speaking, you’re in one of these stages. Just as a reminder,
the stages are:

- Pre-Contemplation: When you’re not active, not thinking about it, and really don’t see why you should be;
- Contemplation: When you’re thinking about getting active, but not quite ready to get around to it yet; and
- Preparation: You’ve thought, you’ve decided, and you’re making arrangements as you read this – you’re just about to start, honestly!

PRE-CONTEMPLATION
If you’re at the Pre-Contemplation stage, you don’t really want to change. Others might have said you should, or you may have read something about why activity’s supposed to be good for you, but deep down, you’re not convinced. As far as you’re concerned, there’s nothing wrong with your life exactly as is.

If this sounds like you, I invite you to take a candid look at your life. Are you truly happy with how things are? Look back over where you are now compared to where you were ten years ago. If the same trend continued for another ten years, would you honestly be happy with where you’d end up?

If so, congratulations! You’re the only person who knows what’s right for you, and no-one, including me, can tell you how you should feel about it. If there’s nothing you’d like to change about your exercise habits, you probably don’t need to be reading this article. If you find, however, that there are things about your life that could be better, let yourself think about them. What’s not exactly the way you want it?

What would your life look like if it *was* the way you wanted it? What would you feel like? You don’t have to do anything right now to change – just let yourself think about what could be better.

CONTEMPLATION
If you’re at this stage, you know you want to make a change, and you’re thinking about what your life might be like if you make it. You’re not quite ready yet though, and that’s OK. Instead of beating yourself up for not having started yet, take a deep breath and give yourself a chance to really explore why you want to get more active.

What exactly do you want? To become healthier? Stronger? To slim down or tone up? Why do you want it? What difference will it make in your life? These are questions that will help to make it easier to stay motivated in future.

Think about how you could turn what you want into a specific, measurable goal. Ensure sure that goal is big enough to inspire you, but realistic enough that you don’t believe it’s doomed to failure from the beginning. If you have a *really* big goal (for example, to lose a large amount of weight, or to compete in a sporting event), think about breaking it down into a series of smaller goals.

Consider starting a fitness journal to answer these questions (if you already keep a journal, just write them in there). Once you’re sure you’re crystal clear on why you want to exercise, you’ll find yourself moving naturally into the next stage – your ‘why’ will drive you to thinking about the ‘how’

PREPARATION
At this stage, you’ve decided that the need for change is stronger than the need to stay the same, and you’ve started thinking about how to make it happen. You might contact a gym, call a personal trainer, or just decide to go for a regular walk.

If you’re here now, it’s time to start exploring how you’re going to make your goal happen. There are many ways to be active, and it’s important to find what’s right for you. The quickest way to kill your motivation is to just throw yourself into the first exercise programme you come across and expect willpower to keep you going. Willpower may be a factor, but
your programme also needs to be enjoyable, convenient, and work for your budget. Some possibilities you may want to consider include:
- Gym classes
- Dance
- Martial arts
- Mind/body activities like yoga or t’ai ch’i
- Walking/hiking/running
- Cycling
- Swimming
- Weights training

Record your research and thoughts in your fitness journal. If you can’t find anything that sounds like it will be convenient, enjoyable and affordable, think about consulting a fitness professional. Teaching A Horse To Ground Tie

Teaching a horse to ground tie can be extremely rewarding and useful. It is similar to the sit and stay command used for dogs.

To train a horse to ground tie, some people tie a rope to a heavy concrete block or an iron weight, or even drive a heavy metal stake into the ground with a fastening ring on it so the horse cannot move, however this method poses risks of injuring a horse if they panic and flee with a heavy weight in tow. An excellent alternative that works very well is just adding a 10 pound sandbag to the end of the lead rope. A Yoga sandbag works perfect, they come with a zipper to add your own sand, dirt, or even grain to the bag, and are usually available in either cotton or nylon with a nylon handle which your lead rope can be easily attached. If you shop around on the web you can usually find them for around $10 or less.

The advantage of the light sandbag is that it’s not likely to injure the horse if he runs off, the worst case scenario is your bag might get ruined, and the bag is just heavy enough if they attempt to walk off it will give a slight tug on the halter as a reminder which is usually enough for a horse that has already been properly halter trained . If a horse runs or walks off, most horses won’t like this bag following along and often will just stop on their own, some may get scared of the bag and panic, but they will eventually get tired and stop running and realize the bag won’t chase them anymore if they stand still.

Attach the sandbag to the end of a 6 to 8 foot lead rope and fasten the other end to the halter. Pick up the sand bag along with the lead rope and lead him around, he should walk beside and slightly behind you, NEVER allow him to get ahead of you. Then drop the rope (and sand bag) to the ground, always make a habit of showing him the rope and practically throwing it to the ground, so he knows that you meant to put it there, and not just dropped it by accident. Then give the command “STAY”, and then walk a couple paces away.

Command the horse to “STAY”, then step back a few feet. If he stands still for a few moments, reward him with a pat on the neck or a treat. However, should he instead attempt to move or follow you, give a firm tug on the lead rope and say “STAY” again, then step back again. For now, just continue to step only a few feet away so you’re still within range to quickly correct him should he decide to move. Once you can get him standing still for a couple minutes at a time, then try gradually moving further away. When you’ve gotten to where you’re now more than 8 or 10 feet from him you’re no longer in range to quickly correct him by tugging on the lead rope, but hopefully by this point when he feels the slight tug caused by the sandbag he will stop and stand still. When tries to move or walk off when you’re at a distance, still yell out the command to “STAY”. If necessary, walk back up to him and place him back in the spot you want him to stand, and give a slight tug on the lead rope and give the command “STAY”, then walk away again. This process takes patience and time and every horse is different when it comes to learning, it make take daily sessions for a week or more. Your goal is to get him to eventually stand still when you drop the rope for at least 5 to 10 minutes while you walk completely away and do something else, such as even going off to retrieve some tack from the barn.
